Evaluation of the Usefulness of ACS: CentaurTM for the Tests of Serum Cardiac Markers

Background :Measurement of creatine kinase isoenzyme MB (CK-MB) in serum has long been recognized as the corner stone of biochemical diagnosis of acute myocardial infarction. Electrophoresis, two-site immunometric assays with separation on solid phase using either monoclonal-polyclonal or monoclonal-monoclonal combinations of antibodies, and direct? immunoinhibition with polyclonal antibodies have been used to measurement CK-MB. The purpose of this study is to compare different methods for the measurement of serum cardiac creatine kinase isoenzyme (CK-MB) Method :CK-MB Rapid Electrophoresis Analyzer-REP system (Helena Laboratories, Beaumont, TX) was used as the reference, Two principles of immunoassay-solid phase separation and immunoinhibition were included in the evaluation. Solid phase separation were from IMx STAT CK-MB (Abbott Laboratories, IL, USA) and OPUS CK-MB (Behring Diagnostics Inc. , MA, USA) and direct immunoinhibition technique from CK-MB Ektachem slide (Eastman Ektachem, NY, USA) . We compared four methods for the measurement of serum CK-MB in 28 normal volunteers and 18 patients with acute myocardial infarction or with post-coronary artery bypass graft. Result :The precision study revealed acceptable total CV% for CK-MB isoenzyme with IMx, OPUS and Kodak Ektachem analyzer. Results for CK-MB obtained with two solid phase separation (IMx, R=0.989; OPUS, R=0.980) , immunoinhibition(Ektachem, R=0.959) showed good correlation with electrophoretic CK isoenzyme. The comparative data for the methods showed that IMx and OPUS correlate well with each other (R=0.985) . Conclusion :Both immunoenzymatic assay that measures mass of enzyme, and immunoinhibition test that measures enzyme activity showed good correlation with electropheresis. CK-MB test should be available 24 hours a day, 7 days a week to diagnose patients with suspicious acute myocardial infarction . Both immunoenzymatic and immunoinhibition assays could be used in diagnosing acute myocardial infarction with ease.
